According to new Federal requirements, you may receive a copy of your Form 1095-C upon request.  This means that The Onslow Co ABC Board is no longer required to automatically send you a 1095-C.

If you need us to mail your Form 1095-C, please contact us as noted below. We will send your form no later than 30 days after we receive your request.

If you are no longer employed by The Onslow Co ABC Board or retired in 2025, you will receive your Form 1095-C in the mail.

Employees can find their electronic 1095-C form. Employees should login to the State Health Plan Benefits Hub on the Benefitfocus platform (aka eBenefits, BenefitPlace, etc.). Then, they hover over the “Benefits” tab and choose “1095 Tax Documents” from Benefit Reports.
